The Gold Coast Health Plan is locally operated so that Medi-Cal recipients can select a primary care physician familiar with Ventura County to coordinate care and services, instead of relying on the state Medi-Cal offi ce to do so remotely. This allows patients to connect with a single provider and reduce unnecessary visits or services through preventive and continuous care. As part of a fi ve-year contract with the Ventura County Medi-Cal Managed Care Commission, ACS handles a range of administrative processes to improve medical access and outcomes for recipi-ents enrolled in the plan. Caseworkers can identify and locate cli-ents and providers closest to their cur-rent location and utilize a new method of documentation via a mobile appli-cation, eliminating the need to com-mute back to the offi ce for data entry. Further, this new application assists the investigation process by presenting clients in a map or list view grouped by due dates and allowing a drill down for further detail. It also identifi es emer-gency caregivers in the vicinity of the caseworker. Investigators are able to record the participants, purpose, time, date and location of a meeting as well as capturing a voice note, picture or video. Once complete, investigators can submit their log through the iPhone app, and it can immediately be viewed by all required stakeholders in the system.
